Admissions and Referrals

  • At California Rehabilitation Institute, we've made the admissions process as simple as possible for our patients, their families, and referral sources, including physicians, case managers and insurance or managed care companies. Our goal is to enable patients to begin their rehabilitation as soon as they are medically stable and able to participate in our highly specialized programs of care. Patients have the right to choose their rehabilitation provider and can be referred directly by their physician, social worker or family member.

    Patients and family members may speak with someone in our admissions department by calling 424-522-7100. Referrals may also be faxed to 424-522-7110. If you or your loved one are in an inpatient hospital setting, you can also request that one of our clinical liaisons visit you for an admission evaluation.

    We will carefully review your medical records and determine admission based on medical history and needs; the ability to participate in 15 hours of therapy over seven days or three hours per day in a five-day period; and individual rehabilitation goals. We help patients determine the best level of care for their recovery, and make recommendations based on our evaluation.
